SCOPE OF POSITION
The Appalachian Highlands Care Network is an innovative, evidence-based health care program that improves health outcomes for low-income, high-risk patients. In our model, Community Health Navigators provide structured advocacy, social support, and navigation to high-risk patients in order to help them reach their health goals. Community Health Navigators are trained laypeople who share life circumstances (race, socioeconomic status, language, etc.) with their patients. They are “natural helpers” within the community. The manager is an energetic, organized leader with experience in community engagement or clinical social work, and case management to manage team of Community Health Navigators

EDUCATION AND EXPERIENCE
Bachelor's degree required, BSW or MSW degree preferred. Completion of a Ballad Health provided Community Health Worker training program upon hire as well as a Community Health Navigator or Community Health Worker Manager training program. Leadership experience with a non-profit or community-based organization preferred.
Valid Driver’s License
If applicable, valid and active Social Work licensure in appropriate state
