Social Worker/ LMSW /Care Transition Manager

Work location: Texas Health Denton 3000 N I-35 Denton, TX 76201 (Onsite)

Work hours: Full-Time, 40 hours weekly, Monday through Friday with periodic weekend coverage

Here's What You Need

Master's Degree Social Work is required upon hire. (Individuals hired as a CTSW prior to May 11, 2017 will be grandfathered to the CTSW position with BSW, at the entity they were employed at on May 11, 2017)

Three (3) years of hospital/medical social work experience is highly preferred.

One (1) year of discharge planning/care management experience is preferred.

Licenses and Certifications

LMSW - Licensed Master Social Worker or LCSW - Licensed Clinical Social Worker is required upon hire.

CPR - Cardiopulmonary Resuscitation is required upon hire.

ACM - Accredited Case Manager is preferred upon hire.

CCM - Certified Case Manager is preferred upon hire.

Other ANCC is preferred.

Skills

Working knowledge of medical necessity criteria preferred

Knowledge of Microsoft Outlook and Office (Word, Excel)

Customer service skills

Ability to engage in complex clinical decision-making

Strong oral and written communication skills

Strong commitment to interdisciplinary collaboration

Critical thinking, analysis and conflict resolution skills

Flexible scheduling as necessary

Psychosocial and crisis intervention skills

Ability to prioritize and meet deadlines

What You Will Do

Delivers care to patients utilizing the Nursing Process

Responsible for ensuring patients are transitioned to appropriate levels of care in a timely and effective manner.
