+ Education: 4 Year / Bachelors Degree, required. Degree in Social Work, Counseling, Divinity, Nursing, or related field, required. Graduate Degree, preferred.

+ Experience: Minimum five years experience in healthcare. Ability to train and monitor advance care planning facilitators, required. Minimum two years exp related to advance care planning, patient advocacy, or medical ethics. Respecting Choices ACP or equivalent and Most/Post form Education, preferred.

+ Additional Skills (required): Content knowledge of advance directives and other advance care planning mechanisms such as MOST/POST/POLST. Experience in program development and implementation in partnership with others, including physician, nursing, and practice manager and case manager leaders. Demonstrated commitment to improving care for patients, including the seriously ill and those nearing the end of life. Knowledge of principles, concepts and methods of health care ethics. Must be able to analyze practical problems, apply a consistent methodology and collaborate with teams working to promote advance care planning. Strong interpersonal, verbal and written communication and advocacy skills required. Ability to work well with all disciplines on the health care team. Possesses tolerance for ambiguity. Demonstrated understanding of clinical processes and problem solving. Ability to drive/travel to multiple facilities/locations as needed.
