Position Summary:
Manages day-to-day operations of admissions office. Position is responsible for receiving telephone calls on all potential residents. Accurately collects data on all potential residents. Schedules and facilitates screenings and/or admissions. Ensures that all admission paperwork is completed in a timely and accurate manner. Handles calls requesting information about the Nursing Home and its programs. Helps foster a positive image of Claiborne Medical Center through public relations and communication within the Covenant Health system and the community. Works with clinical and medical staff to allow clients to be dispositioned to the most appropriate clinical setting.

Responsibilities
+ Manages day to day operations of the Admissions Office and answers all inquiries regarding Claiborne Medical Center. Manage the occupancy levels and quality mix to meet the prospective payment system regulations.
+ Knowledge of policies and procedures to meet government regulations.
+ Demonstrates competence in performing critical skills to include appropriate delivery of care according to the age specific needs of the population.
+ Maintains current waiting list by adding and deleting names per policy. Mails quarterly update letters and make follow up telephone calls when letter is not returned to ensure proper notification.
+ Screens potential residents on the basis of bed availability and will report directly to the Director of Nursing for admission clearance. May accompany the Director of Nursing or the Nursing Supervisor as needed to other facilities to screen patients for possible admission. Respond to inquiries from hospital discharge planners.
+ Assists with orientation of new residents and their families to the nursing home. Ensures room is ready prior to admission.
+ Assists families with process of pre-admission: application, financial arrangements, explanation of resident’s rights, and explanation of advance directives (including living wills, durable power of attorney, and DNRs).
+ Thoroughly completes as much as possible admissions paperwork after all clearances are given and notifies charge nurses and floor staff of impending admission.
+ Obtains social and financial history for all incoming residents and maintains progress notes as required by State, Federal, and the Joint Commission regulations.
+ Provides on-going clinical casework to residents and families, monitoring daily psycho-social, emotional, and physical adjustments.
+ Develops a partnership with the clinical team to ensure that the clients are being given the proper treatment.
+ Refers needs/problems of residents to family, staff, physician or appropriate department.
+ Liaison person between resident and Ombudsman for specific problems in conjunction with social services.
+ Assists in discharge planning for short-stay residents as well as other residents who wish to be discharged from this nursing home. Determines the need for or makes arrangements for home health or medical equipment as ordered by the physician.
+ Monitors discharged residents when appropriate and as required by physician.
+ Checks the facility complaint log each shift while on duty and follows up on all complaints/concerns.
+ Functions as a member of the interdisciplinary team and completes appropriate sections of the MDS and helps to develop and review care plans.
+ Leads Family Advisory Council which serves as a source of information and support to family members.
+ Represents Claiborne Medical Center on the PPS Team and the Triple Check Team and will attend the monthly meetings of the Admissions Affiliate group of THCA.
+ Coordinates with Marketing to ensure that the Referral Service is up to date.
+ Conducts Nursing Home resident activities in the absence of the Activity Coordinator.
+ Assists in maintaining a safe environment for all residents, which may include reporting to appropriate managerial staff equipment that is out of order, suspicion of resident abuse, mistreatment, neglect and reporting of suspicion of criminal activities.
+ Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
+ Performs other duties as assigned.
Qualifications
Minimum Education:
None specified; will accept any combination of formal education and/or prior work experience sufficient to demonstrate possession of the knowledge, skill and ability needed to perform the essential tasks of the job, typically such as would be equivalent to an Associate’s degree. Preference may be given to individuals possessing a Bachelor’s degree in a directly-related field from an accredited college or university.
Minimum Experience:
One (1) year of customer service experience working with geriatric patients and families required.Nursing home billing experience, accounting skills, and worked with Excel software preferred.
