**Responsibilities:**

+ The Faith Community Nursing (FCN) Coordinator supports the development, implementation, and evaluation of faith community nursing programs in assigned communities.

+ Serving as a liaison between the healthcare system, local faith communities, and the community at large.

+ The coordinator guides both paid and volunteer FCNs in providing holistic care that integrates physical, emotional, and spiritual well-being.

+ Key responsibilities include recruiting and orienting FCNs, assessing health needs, coordinating and performing wellness screenings and education events, as well as presentations to nurses and clergy.

+ The coordinator also maintains program data, ensures compliance of data collection from FCN Volunteer and self reporting on Community Benefit Form.

+ Fosters partnerships with congregations and community organizations to align services with BayCare’s health priorities and Community Health Needs Assessments (CHNAs).

+ The faith community nurse coordinator is responsible to coordinate care of the clients enrolled in the FCN Transitional Care Program.

+ Responsible for transportation between facilities and to community events.

+ Other duties as assigned.

**Minimum Qualifications:**

**Required** **Education:** Associate's degree in Nursing or Diploma Nursing. **Preferred** **Education:** BSN (Bachelor of Science in Nursing).

**Required Certification:** BLS (Basic Life Support) and RN (Registered Nurse).

**Required Experience:** Minimum 3 years of direct patient care experience. Working knowledge and computer proficiency (Microsoft suite).
