**Job Summary and Responsibilities**

**Position Summary:**

Participates as a member of the health care team, working with professional and non-professional staff and associated professionals in providing care which provides optimum home care and continuity of care to referred patients, addressing the age of the patients served.

**Minimum Qualifications** :

+ Current CA Physical Therapy licensure

+ BLS CPR certification (Must be through the American Heart Association)

+ 1 year experience as a physical therapist

+ Knowledge and ability to perform current physical therapy techniques

+ Knowledge of physical therapy standards procedures and principles

+ Ability to operate physical therapy equipment for patient care

+ Ability to implement practical application of knowledge

+ Ability to work independently and autonomously

+ Ability to teach and instruct patients caregivers and staff

**Preferred Qualifications:**

+ 6 months to one year home health experience

**Pay Range**

$47.52 - $70.68 /hour
