INPATIENT DISCHARGE PLANNER

STARTING RATE WITHOUT EXPERIENCE: $23.55

JOB SUMMARY

+ The Discharge Planner partners with the Unit C3, nursing, Social Worker and providers to ensure a discharge plan is created and keeps the care team, patient and family updated on the discharge plan. It is their responsibility to be knowledgeable of discharge and pay specific utilization criteria in order to ensure patients’ needs are fully met with resources at the time of discharge. The Discharge Planner will also be aware of the continuum of care beyond the patient’s inpatient stay.

This position requires full understanding and active participation in fulfilling the Missions of Blessing Health Systems. ICARE standards must be demonstrated. Adherence to Code of Conduct and other policies is required
