**Responsibilities**

**Job Summary:**

**The Community Health Advocate will work primarily within the healthcare provider offices as well as with Emergency Department, community, at client homes, hospital case managers, and with other agencies/organizations to coordinate care and client needs. Works as a clinic team member focused on care coordination.**

**Essential Functions:**

+ Conduct health education and screening outreach in community and healthcare provider settings in order to provide health education and raise awareness of health concerns and available community resources and services.

+ Reduce client barriers to health care and needed community resources and services.

+ Facilitate communication and coordination between community resources/services and providers.

+ Maintain knowledge and understanding of available community resources, services, and programs.

+ Create connections between vulnerable populations and needed community services and resources.

+ Provide health screenings for conditions and diseases as well as providing health information on those condition/diseases and areas of concern.

+ Assist with establishing a unified referral system, as well as evaluate and improve the community health workers program.

+ Works closely with Providers and care coordination teams.

+ Help patients develop health management plans and goals.

+ Coach patients in effective management of their chronic health conditions.

+ Other duties as assigned by management.

**Qualifications**

**Required Education and Experience:**

+ High school diploma or GED

+ Prior community outreach or medical/office/heathcare experience

+ Prior experience with teaching or one on one (1:1) counseling for health concerns

**Required Licensure and Certifications:**

+ Community Health Worker Certification within three (3) months of hire

+ Current AHA Basic Life Support Certificaiton

**Required Minimum Knowledge, Skills and Abilities** :

+ Language Ability: Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to common inquiries or complaints from groups of managers, clients, customers, and the general public.

+ Math Ability: Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

+ Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

+ Computer Skills: Must be proficient with the Microsoft Office Suite, Internet navigation, database management, spreadsheet, E.H.R., and graphic presentation packages.

**Preferred Qualifications:**

+ Previous experience

+ In-home visitation experience and phone follow-up experience recommended

**Pay Range**

$17.89 - $24.60 /hour
