+ **Care Coordination:** Facilitate interactions between patients and their clinical care team, ensuring a smooth and coordinated experience throughout the continuum of care.

+ **Patient Education:** Empower patients, families, and caregivers by educating them about diseases, treatments, side effects, and how to access reliable information.

+ **Resource & Support:** Collaborate with providers to implement care standards, and identify/secure appropriate services.
