**RN Case Manager**

As our RN Case Manager, you will help patients navigate complex healthcare journeys, ensuring seamless transitions, advocating for their needs, and coordinating resources to optimize their health outcomes and promote continued well-being. Every day you will assess patient needs, develop comprehensive care plans, and facilitate the delivery of appropriate services across various care settings. You will be expected to manage and supervise care plans according to policies, procedures, and the State Nurse Practice Act. To be successful in this role, you must be a skilled communicator, an expert problem-solver, and a compassionate advocate, dedicated to patient-centered care and resource management while adhering to established standards.

**Responsibilities**

+ Conduct comprehensive assessments of patients' medical, social, psychological, and financial needs to identify barriers to discharge and develop individualized care transition plans.

+ Coordinate and facilitate the delivery of appropriate healthcare services, collaborating with physicians, nurses, social workers, insurance providers, and community resources to ensure continuity and quality of care.

+ Advocate for patients and their families, ensuring their rights are respected, their preferences are considered, and they are empowered to make informed decisions about their healthcare journey.

+ Manage patient caseloads, monitoring progress, evaluating the effectiveness of interventions, and making necessary adjustments to care plans to achieve optimal patient outcomes and efficient resource utilization.

+ Educate patients and families on their health conditions, treatment options, available resources, and discharge instructions, preparing them for successful transitions to home, rehabilitation, or other care settings.

+ Maintain accurate and timely documentation of all case management activities, assessments, interventions, and communications, ensuring compliance with hospital policies, regulatory requirements, and payor guidelines.

**& THIS JOB OFFERS BOTH AN INCENTIVE AND RELOCATION:** We are offering a **$5,000 Sign-On Bonus** in addition to relocation to new hires who meet the eligibility requirements.

**Union Work Agreement:**

FTE Status: Full Time - 1.0; 80 hours per pay period

Shift Type/Length: Days, 8 hours

On Call Expectations: None

Weekend/Holiday Frequency: Rotating Weekends & Holidays

**Job Requirements**

**Required Qualifications**

+ Nursing Degree from an approved school of nursing

+ **LICENSURE** Active Registered Nurse license in good standing with the North Dakota Board of Nursing or Active Registered Nurse license in a compact state

+ Working knowledge of patient care across various healthcare settings, care coordination principles, and utilization review

+ Basic Life Support (BLS) Certification within the orientation period and meets re-certification as required

+ Good communication skills necessary for constant communication, both oral and written, with patients, families, visitors, and medical center staff

+ Accountable for own conduct and promotes and supports positive working relationships among staff, between other disciplines, departments, families, visitors, and patients

+ Maintains confidentiality

**Pay Range**

$35.38 - $50.69 /hour
