As a counselor for Treatment Associates, maintained a case load of up to 70 patients providing Individual counseling services to those in opiate replacement treatment program; occasionally upon the patient's request, con joint and family counseling sessions were conducted as needed. Duties included completing initial intake process and treatment plan for all patients assigned to my case load as well as providing chemical dependency education and linking patients with community resources that match their needs. Maintained all paperwork in a timely manner to insure state and federal compliance (PROGRESS REPORTS,ASAM"S, TX PLAN'S, ASI'S, INTERPERATIVE SUMMARIES, RELEASE OF INFORMATION, ETC) Launched and maintained the clinic's first successful group's and work shop's utilizing the Matrix series; facilitated groups on stimulant abuse, anger management, relapse prevention, early recovery skills as well as general process groups; utilized and monitored successful, abstinent patient's to co facilitate patient led groups. Subsequently voted employee of the quarter due to the success of the groups (ua's were tracked and pt's who participated stayed negative) and consistently being top billing counselor, nationwide. Processed pt urine samples per state lab guidelines on a random basis or when a patient was suspected to be under the influence. Assessed and referred patients as needed; Met with treatment team on a weekly basis to ensure quality of patient care and case compliance; also worked with other clinicians in regards to patient treatment for issues outside of scope of practice; other duties as required.
