Position Title: Associate Community Dementia Program Manager

Position Summary

The Bilingual Community Dementia Program Manager plays a vital role in supporting Sibley’s innovative Club Memory® program — a community-based initiative for individuals living with early-stage dementia and their care partners. This role combines program coordination, data management, and hands-on support to deliver high-quality dementia-specific services, education, and resources that promote cognitive health, well-being, and connection.The Bilingual Community Dementia Program Manager will also communicate with non-English speaking patients to ensure accurate communication, understanding and comprehension.

Key Responsibilities

  • Partner with Community Dementia Program Managers to plan, implement, and evaluate Club Memory® and related support programs.

  • Conduct interviews and assessments to determine member eligibility and dementia care needs.

  • Lead data collection, analysis, and reporting to drive program improvement and demonstrate outcomes.

  • Support the development of psychosocial and educational interventions for members and care partners.

  • Facilitate dementia-friendly group activities such as music, exercise, and memory engagement sessions.

  • Assist in connecting members and families to appropriate community and healthcare resources.

  • Maintain accurate program records, attendance, and communication logs.

  • Deliver dementia education and awareness training for community members, families, and professionals.

  • Advocate for members and care partners to ensure access to quality support and resources.

  • Contribute to grant applications and help manage program supplies and materials.

Qualifications

  • Bachelor’s degree required.

  • Bilingual in Spanish

    • Candidates must successfully pass the required JHM approved designated language assessment prior to job offer.
  • Strong interest in program evaluation and data analysis.

  • Background in nursing, social work, counseling, family therapy, or recreational therapy preferred.

  • Experience with dementia care or programming for older adults highly desirable.

  • Exceptional communication, assessment, and public speaking skills.

  • Ability to work collaboratively, multitask, and maintain professionalism in a dynamic environment.

Salary Range: Minimum 32.14/hour - Maximum 38.46/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
