SUPERVISOR PARTIAL HOSPITALIZATION PROGRAM

STARTING RATE WITHOUT EXPERIENCE: $30.78

*UP TO $20,000 SIGN ON BONUS AVAILABLE*

JOB SUMMARY

This position is responsible for providing leadership and supervision in program development and marketing of Partial Hospitalization Program and Intensive Outpatient Program services as well as managing the full scope of utilization review and outcomes tracking for the programs to the Behavioral Health Service Line. This position requires assessment to one or more age populations, including adolescent through geriatric, in a manner that demonstrates an understanding of the functional/developmental age of the individual served and providing group, individual, and family therapy services. In addition, this position must understand cultural diversity and advocacy expertise that is linked to community resources. This position develops, implements, coordinates, and evaluates patient treatment plans, collaborates with other health care providers, community service agencies, faith-based services as appropriate to coordinator care and services, and maintains and understands regulations relating to the programs.

This position requires full understanding and active participation in fulfilling the Missions of Blessing Health Systems. ICARE standards must be demonstrated. Adherence to Code of Conduct and other policies is required

JOB QUALIFICATIONS

Education/Training/Experience:

REQUIRED:

+ Masters's Degree in Social Work or related degree.

+ Current LCSW/LCPC

+ Experience with Behavior Health Patients.

+ Minimum 1-2 years of experience in leadership and individual/family therapy.

PREFERRED:

+ Three years clinical experience in leadership and individual/family therapy.

License/Certifications/Special Requirements:

+ POSSESSES CURRENT ILLINOIS LICENSE AS A LCSW/LCPC
