Shift: Mon-Thurs 8am-5pm, Fri 8am-12pm

The role you’ll contribute:. The Targeted Case Manager is responsible for conducting a comprehensive assessment and periodic reassessment of an individual’s strengths and needs and assist them in gaining access to identified medical, social, educational, and other needed services and support.

Assist patients in obtaining needed referrals and support based on identified needs

Qualifications

The expertise and experiences you’ll need to succeed:

· Bachelor of arts or science degree in a behavioural science from an accredited university

· One year full-time employment working directly with individuals in a human service setting

· Completion or agree to complete AHCA-approved targeted case management training within three months of initially providing services _Preferred_
