BayCare is currently in search of our newest **Home Health Aide Per Visit** who is passionate about providing outstanding customer service to our community. We are looking for an individual seeking a career opportunity with one of the largest employers within the Tampa Bay area.

**Position Details:**

+ **Facility:** BayCare Health System, Private Duty

+ **Location:** Pinellas County

+ **Status:** Per Visit, hourly

+ **Shift Hours:** 7:00 AM - 7:00 PM, Varies

+ **Weekend Work:** Every 3rd weekend

+ **On Call:** No

**Home Health Aide** key responsibilities:

+ As part of the patient care team, works under the guidance and supervision of the staff nurse in providing personal care to the patients

+ Follows the patient's plan of care for completion of tasks assigned to a home health aide by the registered nurse or other appropriate skilled professional

+ Maintains an open communication process with the patient, representative (if any), caregivers, and family

+ Demonstrates competency with assigned tasks

+ Complies with infection prevention and control policies and procedures

+ Reports changes in the patient's condition

+ Honors patient rights

**Required Certifications and Licensures:**

+ CNA (Certified Nursing Assistant) or HHA certification from accredited 75-hour program

+ Driver's License State of Florida (Florida driver's license is required within 30 days of hire)

+ BLS (Basic Life Support)

**Education:**

+ **Preferred:** High School Diploma or equivalent

**Experience:**

+ **Preferred 1 year Home Care**
