MDS Coordinator- Certified- Transitional Care Unit
PRN/OCC, Day Shift, Work 1-2 Days Per Week, Monday - Friday 8:00am - 4:30pm
Position Summary: Assesses, plans, monitors and evaluates options and services to meet an individual's health needs through communication and the coordination of available resources. Enhances service quality and assists in the delivery of cost effective outcomes in collaboration with health care team.
Minimum Education: None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.
Minimum Experience: Working knowledge of long term care process/procedure and MDS regulations. Familiarity with regulatory requirements, CMS and state license requirements. Two (2) to five (5) years clinical practice is required. Excellent communication skills, leadership skills; ability to accept and incorporate feedback; problem solving and priority setting skills. Preference given to previous MDS Coordinator.
Licensure Requirement: Current RN license, RAC-C (Resident Assessment Coordinator-Credentialed) certification through AANAC required within 90 days of hire into position.
