JOB SUMMARY: Provides patient and family advocacy; clinical psychosocial assessment, diagnosis and appropriate clinical social work treatment/interventions/discharge planning to patients/ families identified as having significant social, emotional, mental and substance abuse issues. Strives to promote patient/family wellness, improved care outcomes, and access to appropriate and hospital and community resources for assigned patient population.
Required Education: BSW or BS/BA in related field
Preferred Education: MSW
Required License: Driver's License if required
Preferred License: LCSW
Required Experience: At least two years'experience providing clinical psychosocial assessment in medical setting
