+ **Shift Hours:** 8:00 AM - 5:00 PM

+ **Weekend Work:** Occasional

+ **On Call:** Yes

+ **Home visits in assigned service area:** Dunedin, Palm Harbor, Tarpon Springs, Oldsmar, and surrounding areas

The Home Health Field, RN provides **hands-on care** in the home for a group of patients consistently using the nursing process and clinical standards and protocols. **Responsibilities include** :

+ Oversee the activities of the multi-disciplinary team that provides care to the patient

+ Monitor the provision of therapeutic services through written and verbal communication with all disciplines

+ Ensure that all visits scheduled and provided are authorized and covered by physician orders

+ Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits

+ Inform the scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor

**Home Health RN Requirements:**

+ BLS (Basic Life Support)

+ RN (Registered Nurse) License

+ Driver's License State of Florida (Florida driver's license is required within 30 days of hire)

+ Required Associate's or Diploma Nursing

+ Required 1 year Registered Nurse

+ _Preferred_ Bachelor's Nursing

+ _Preferred_ 1 year Home Care
