Job Description


JOB SUMMARY: Coordinates the plan of care to skilled nursing and assisted living facilities and other post-acute providers and community resources in a cost-effective and time efficient manner. Serves as a true patient advocate and a resource for Care Management Team, multi-disciplinary staff and physicians. Collaboratively engages with Care Management staff to ensure appropriate utilization of resources and smooth transition for facility placements in the community and region. Collaborates to achieve optimal patient satisfaction and positive clinical, financial and quality outcomes for assigned patient population across the system.

REQUIRED EDUCATION: BA/BS is related life/health science field.

PREFERRED EDUCATION: BSW

REQUIRED LICENSE: None

PREFERRED LICENSE: Level I: NC Certified Social Worker, Accredited Case Manager (ACM)

REQUIRED EXPERIENCE: 3 years' experience in health care setting or local social service agency. BSW internship at Mission or other acute care hospital accepted in lieu of required experience.

PREFERRED EXPERIENCE: 5 years hospital/healthcare discharge planning experience.
