This is a PRN position that provides coverage for our two Adult Day Centers: Henderson and Brevard.

Job Summary: Under the oversight of a Registered Nurse, the LPN Health Care Coordinator is responsible for the delivery of health care services and participates in direct care for the participants attending Adult Day Services, using a holistic and Person Centered Approach. Knowledge and understanding of the physical and emotional aspects of aging is necessary. He/She must be able to identify emergency situations, carry out emergency measures as indicated and recognize issues occurring with the participants to determine their health/medical situation to determine if they are able to remain at the center. He/She will be flexible, patient, creative, and use good problem solving skills. He/She works closely and effectively with participants and staff and represents Adult Day Services in a positive and professional manner. He/She participates in the team process as an active team member, as well as independently. He/She is responsible for delegating nursing care tasks to qualified unlicensed personnel as necessary.

Required Education: High School Diploma/GED; graduated from an LPN program; CPR; First Aid

Preferred Education: at least 2 years of college

Required License: Licensed Practical Nurse

Required Experience: Six months to one year related experience

Preferred Experience: Working with impaired adults
