Job Summary: The Patient Navigator is a specialized health care professional who follows patients with a specific medical diagnosis or healthcare need exits. The Patient Navigator will focus on the following areas of service: clinical services, education, care coordination, data and research management. The individual acts as a liaison for the patient and family addressing their concerns and questions. Additional responsibilities include pre and post follow-up with patients and facility. Serve an educator and liaison for the healthcare team.

Required Education: Bachelor of Science in Nursing. Nurses with an Associate Degree or Diploma in nursing must sign agreement to obtain BSN within 6 years of the hire date in to position. Nurses who are nearing the end of their professional career may be exempted from this requirement with CNO System Council approval.

Required Education: Bachelor of Science in Nursing or Radiology Sciences specializing as Interventional Radiology Technologist

Preferred Education: BSN; or BS in Radiology Sciences with Certification in specialty recognized national nursing organization and/or Interventional Radiology

Required License: Current licensure as an Interventional Radiology Technologist or Registered Nurse with North Carolina Board of Nursing or compact state as defined by NC Board of Nursing. BLS Certification.

Preferred License: ACLS, other certification as required by specialty

Required Experience: Minimum of 3 years nursing experience or 3 years IR experience

Preferred Experience: 5 years nursing or Interventional Radiology experience
