Director - Care Management - Case Management - Orlando
Florida Hospital Orlando seeks to hire a Director - Case Management who will embrace our mission to extend the healing ministry of Christ.

Facility Profile:
Located on a lush tropical campus, our flagship hospital, 1,107-bed Florida Hospital Orlando, serves as the major tertiary facility for much of the Southeast, the Caribbean and South America. Florida Hospital Orlando houses one of the largest Emergency Departments and largest cardiac catheterization labs in the country. Department Profile:
The goal of Case Management is to promote quality patient care via effective discharge planning and efficient, value-based utilization of health care resources. The case management team provides a comprehensive and collaborative process which assesses, plans, intervenes, facilitates, and advocates for the needs of patients and their families.

Work Hours/Shifts:
Monday - Friday; 8a - 5p

Job Summary:
The Director - Care Management - ORL works to plan, organize, implement, and evaluate the medical management/case management program at Florida Hospital. This includes capacity and LOS management, discharge planning, social work, and utilization management. The Director works with the Administrative Director, staff, and directly with campus administration to facilitate both system-wide and campus-specific capacity and LOS initiatives. The Director oversees the care management staff in planning, implementing, and re-evaluating the psychosocial and discharge planning needs of emergency department, neonatal, pediatric, adolescent, adult, and geriatric patients. Adheres to Florida Hospital Compliance Plan and Utilization Review Plan for the hospital in accordance with federal/state guidelines, accepted practice, hospital policy, accreditation bodies, and in cooperation with administration, the medical staff, and all operational hospital departments. Actively participates in assuring outstanding customer service and accepts responsibility for maintaining relationships that are equally respectful to all.

  • Leadership experience in case management, managed care, long-term care, home health care (Preferred)
  • Experience in developing and overseeing programs that address capacity and length of stay issues (Preferred)
  • Experience in discharge planning in acute care setting (Preferred)
  • For Nurses: Bachelor's degree in business or health care discipline
  • For Social Workers: Master's degree in Social Work
  • Bachelor's degree in Nursing (Preferred)
  • MSN, MBA or MHCA (Preferred)
  • Recent acute health care experience, minimum of two years

Licensure, Certification, or Registration Required:
  • For Nurses: current, valid State of Florida license as a registered professional nurse
  • For Social Workers: current, valid State of Florida license as a clinical social worker
  • CCM or ACM (Preferred)

Job Responsibilities:

  • Facilitates campus and system initiatives to achieve capacity and LOS goals, Avoidable Delays, and other Care Management initiatives. Serves as campus leader for Avoidable Delay program.
  • Oversees management of the financial aspects of departmental operations
  • Develops and maintains liaisons with local, state, and federal resources. Applies knowledge of local, state, and federal programs to oversee and expedite discharges while assuring appropriate stewardship of healthcare resources.
  • Oversees provision of the discharge planning/social work activities based on age specific need of the patient/family.
  • Assists with the ongoing, campus-wide case management performance improvement program, ensuring a single standard of care while working within individual campus requirements.
  • Oversees management of the financial aspects of departmental operations and is accountable to departmental, campus and system budgetary guidelines.
  • Participates in campus- and system-hospital quality improvement initiatives in relation to general hospital business outside of Care Management
  • Participates in campus and system clinical, team, financial and market strategies for campus/system element development
  • Develops, implements, monitors, and evaluates departmental goals and objectives with campus ancillary departments and physicians and ensures campus compliance with campus/system LOS goals.
  • Assist ancillary departments with creation of policies and procedures applicable to patient discharge/transfer while including interdisciplinary procedures/requirements.
  • Develops and maintains Care Management policies and procedures consistent with general hospital policy and including any applicable federal, state, and local laws (maintain up-to-date knowledge of current regulations/laws).
  • Attend and/or lead and/or participate in department, campus, and system meetings to facilitate specified goals.
  • Provide 24-hour/day, 7-day/week rotational on-call management coverage for all campuses to facilitate all responsibilities/activities mentioned herein.
  • Facilitate and assists Risk Management in the CMS Grievance and Mediation Process.
