Florida Hospital Altamonte seeks to hire Lead Care Manager who will embrace our mission to extend the healing ministry of Christ.

Department Profile:
The Care Management Department is located in every campus throughout the Florida Hospital system. Our objective is to create a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for services to meet an individuals and family's comprehensive needs. Care Management is an integrated department with physicians, clinical/nursing teams, ancillary departments and the revenue cycle teams. Care management meets the needs of our patients, families and significant others throughout the care continuum including the post-discharge needs.

Work Hours/Shifts:
Weekends; 8a - 6:30p / 10 hr shifts

Job Summary:
The Lead Care Manager (LCM) is a care management professional who supports the Care Management Manager and/or Director in overall operations of the Care Management staff. Exhibits critical-thinking skills, creativity, and manages resources to achieve quality performance, positive employee relations and compliance with hospital guidelines. The LCM staffs, trains, mentors and facilitates workflow processes within the care management team in the delivery of services to our patient population. The LCM ensures quality care is delivered to our patients and effective stewardship of scarce resources. Supports change activities and works to ensure implementation of change that may occur. In the absence of the Director/Manager, escalates all issues up to their immediate supervisor. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Knowledge, Skills, Education, & Experience Required:
  • Ability to mentor the care management team
  • Ability to integrate performance improvement initiatives within Care Management department under the direction of the leader
  • Ability to think critically
  • Ability to assist and mentor others in relation to the technology and processes within the care management team
  • Excellent social and clinical practice knowledge with ability to apply this knowledge
  • Ability to influence relationships and behaviors in relation to meeting the needs of the patients
  • Ability to act as a role model for professionalism
  • Ability to act as a liaison to clinical leadership in the absence of the manager or director
  • Ability to problem solve and know when to escalate decision making to the manager or director
  • Basic computer skills such as word processing, data management, internet/email
  • Registered Nurse OR
  • Bachelor's Degree in Social Work (BSW)
  • For Registered Nurses, BSN (Preferred)
  • For Social Workers, MSW (Preferred)
  • Experience in acute care hospital discharge planning (Preferred)

Licensure, Certification, or Registration Required:
  • If RN, current valid State of Florida license as a Registered Nurse li <>
  • Certified Case Manager through the Care Management Society of America (Preferred)
  • Accredited Case Manager through the American Care Management Association (Preferred)

  • Utilizes knowledge of operational expectations for all employees on the unit and exercises independent judgment to make recommendations to supervisor/manager for improvements.
  • Mentors the employees in a manner that develops the employees to their optimal level of performance in order to ensure competency in care management delivery. Develops staff preceptors and mentors.
  • Utilizes familiarity with the performance improvement model of the organization and supports performance improvement activities on the unit through problem identification, assessment, data gathering, change implementation and evaluation of outcomes.
  • Utilizes critical-thinking skills in the performance of their duties as a LCM and utilizes data-driven decision making, incorporating evidence into the decision process and problem solving.
  • Utilizes knowledge of the broad impact of reimbursement on the delivery of patient care services, including but not limited to Medicare, Managed Care, Medicaid, and third-party providers.
  • Exhibits an understanding of the technology utilized in Care Management day-to-day operations
  • Assesses patients for clinical and social needs. Plans for delivery of care management to the patient and evaluates the outcomes of care delivered. Ensures the patient experience is the focus of all services.
  • Monitors and leads staff in relation to established processes and outcome measures. /li>
  • Utilizes active listening, feedback, inquiry, and validation in communicating with others in relation to meeting the goals of care management.
  • Promotes, nurtures, and maintains interdisciplinary relationships in order to ensure patient-focused care and better patient outcomes.
  • Maintains appropriate collaborative relationship with patients, families, staff, vendors, physicians, other disciplines and other contacts.
  • Acts as a role model for professional behavior and maintains professional appearance adhering to departmental dress code at all times.
