Clinical Social Worker III - Center for Behavioral Health - Orlando

Work Hours/Shift :

Full time, Variable Schedule - mostly Tues-Sat, Shifts include days, evenings, nights and weekend.

Job Summary :

The Social Worker III is responsible for the provision of psychosocial services for patients, their families, and patient experience within the Center for Behavioral Health. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Skills, Education & Experience Required :
  • Excellent communication, negotiation and advocacy skills with patients, families, Team members, Health Care Team and community resources
  • Working knowledge of community and treatment resources based on age and clinical needs.
  • Excellent ability in time management, organization skills, and prioritization of job duties in an efficient, timely and responsive manner
  • Scheduling flexibility, working evenings and weekends
  • Master's Degree in Social Work or Counseling.
  • One year experience working with psychiatric or medical patients

Licensure, Certification or Registration Required :

  • None
  • State of Florida Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC) (preferred)
  • Registered Clinical Social Work Intern or Registered Mental Health Counselor Intern (preferred)

Job Responsibilities :

Demonstrates, through behavior, Florida Hospital's core values of Integrity, Compassion, Balance, Excellence, Stewardship, and Teamwork

  • Responsible for achieving and maintaining year round Patient Experience scores in the 75 th percentile or the targeted metric determined by Florida Hospital
  • Responsible for reducing Length of Stay and avoidable delays in accordance with targeted metric established by Florida Hospital Leadership; daily Length of Stay (LOS) tracking and report out to Social Services Supervisor
  • Completes Psychosocial History on each patient within 48 hours of admission, within 24 hours of admission for Medicare patients
  • Completes daily workflow and processes as assigned by the Social Services Supervisor or designee ensuring all tasks are executed in accordance with DNV, DCF, and AHCA standards, as well as, departmental protocols, Florida Hospital Policies and Standard Operating Procedures (SOPs)
  • Facilitates age appropriate therapeutic groups daily on inpatient unit
  • Provides family therapy as indicated for patients on inpatient unit in accordance with department standard/protocol
  • Conducts daily clinical rounding and individual therapy sessions for patients on inpatient unit in accordance with department standard/protocol
  • Implements initial discharge plans on each patient within 24 hours of admission and ensures each patient has completed discharge plans with scheduled appointments as indicated upon release from the hospital
  • Provides daily documentation of clinical and case management services in accordance with department standard/protocol
  • Acts as patient advocate for patient needs within the community.
  • Participates with the interdisciplinary team in developing and implementing a clinical treatment plan.
  • Performs additional duties as assigned or directed to ensure the smooth operation of the department.
