Discharge Nursing Coordinator - East Orlando Strategic Initiatives - East Orlando - Part Time
East Orlando seeks to hire Discharge Nursing Coordinator who will embrace our mission to extend the healing ministry of Christ.

Florida Hospital East Orlando, a 293-bed full service community hospital that serves as one of Florida Hospital's seven satellite facilities, became part of the Florida Hospital system in 1990. Work Hours/Shifts:
M-F, 2pm - 6pm

Job Summary:
The Discharge Coordinator makes post discharge phone calls to both inpatients and Emergency Department patients, answers general questions patients may have, and receives feedback regarding their patient experience. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Knowledge, Skills, Education, & Experience Required:
  • Pleasant telephone manner and organizational skills
  • Proficient computer skills and medical terminology required
  • Critical thinking and ability to prioritize
  • Proficient in Microsoft Outlook, Excel, and Word
  • Bilingual proficiency: English required/Spanish (preferred)
  • Associate's degree in Nursing
  • Minimum of three years' experience with direct patient care: inpatient, acute.
  • Bachelor of Science degree in Nursing (preferred)
  • Experience in risk management, discharge planning or complaint management (preferred)

Licensure, Certification, or Registration Required:
  • Current, valid State of Florida license as a registered professional nurse

Job Responsibilities:

  • Makes telephone contact with patients within 24-48 hours after discharge to assess patient education, compliance with discharge instructions, and patient satisfaction.
  • Serves as a liaison between discharged patients and professional staff and clinical departments. Answers questions regarding discharge instructions and uses available resources to provide immediate service recovery.
  • Uses a centralized database to record and track patients' responses. Supports the clinical managers in recognizing employees deserving recognition as they are identified in the discharge telephone process and performance improvement.
  • Intervenes promptly when issues are reported after discharge. Makes recommendations for both process and performance improvement.
  • Uses key words and phrases to strengthen the overall impression of FH. Serves as a role model for the FH patient experience.
  • Responds to calls from staff and patients to serve as a liaison between clinical/non-clinical staff and patients/families to facilitate two-way communication, support, and information. Facilitates communication for families with the medical team; provides necessary information and comfort measures. Responds to conflict situations with "difficult" patients/families throughout the campus. Coordinates prompt, sensitive and professional response to each concern.
  • Serves as a patient advocate not only for isolated complaints, but also functions as a global patient advocate by using aggregated data to support positive change. Uses downtime to support and recommend performance improvement projects to various units, report trends noted in both feedback from RiskMaster, discharge telephone calls, etc., to the assigned nurses, managers and directors, and supports the customer service initiatives throughout FH.
  • Promotes the mission and vision of the department by serving on various committees and councils that promote performance improvement and customer service.
  • Collaborates with Case Management, physician offices and community resources, as appropriate, to assist the patient with transition of care needs.
