This position is with the PACE Program Job Summary: Under the supervision of the Transportation Coordinator is responsible for the safe and timely transportation of PACE participants. Including transport of participants, meals, durable medical equipment, pharmacy, and any other transportation as deemed necessary. Responsible for overall cleanliness and safety of the vehicle assigned on a daily basis. Responsible for communicating any safety issue to the Transportation Coordinator. Transport participants to and from medical/specialist appointments and activity outings as requested by the interdisciplinary team via the plan of care. Transport meals, durable medical equipment, pharmacy, and any other items as deemed necessary for participant. Provide physical assistance to each participant when needed as s/he boards and disembarks from the program's vehicles. Maintain the vehicle in a neat and clean condition. Ensure vehicle equipment is maintained and operating within manufacturer's recommendations, replacing supplies as needed. Function as a member of the interdisciplinary team when appropriate. Communicate with Transportation Coordinator or designee daily to be updated on participant conditions/changes, plan of care changes, or interdisciplinary team requests. Follow Policies and Procedures and OSHA safety guidelines. Maintain the confidentiality of all company procedures, results, and information about participants, clients or families. Handle potentially infectious fluids (e.g. blood, urine, emesis) with appropriate biohazard precautions and practices Standard Precautions. This position will monitor denials, identify areas of opportunity, and implement self-sustaining changes to improve operational effectiveness and financial gain.The System Manager is primarily responsible for providing direction and oversight of the Billing and/or Denials Administration team while overseeing claim preparation, claim submission and billing edit functionality system wide. Provide functional guidance to subordinate billing and denials staff in supporting area operations and programs ensuring all billing/payer edits and reason/remark codes are continually updated. The System Manager will also maintain appropriate files, reports and other statistical data as required and provide results of all special projects and provide recommendations for additional revenue opportunities. The System Manager is responsible for keeping informed of new changes in billing regulations and denial resolution method. Resolving ways to work within any new regulation and coordinating any testing or move to production efforts as it applies to billing functions. Performance Criteria : Ensures patient flow in the clinic is maintained, and delivers care in an organized manner according to patient needs. Ensures accurate and timely communication to patients, referring providers, and other healthcare professionals. Accomplishes assigned tasks by taking vital signs of patient and completing patient information with knowledge of pharmaceuticals, disease, and common side effects appropriate to the population served. Performs blood draws, administers medication, gives injections, and performs tests as directed by medical provider. Ensures accurate and timely documentation in patient medical record that supports compliance of health care initiatives. Ensures safe environment for patient. Provides patient and family teaching. Participates in on-going assessment process. Performance Criteria : Ensure completion of social and medical history. Ensures equipment and supplies are ordered, exam rooms continually stocked and ready for use. Is competent in techniques, procedures, regulations appropriate to the population served and updated per clinic standards. Ensures personal computer literacy is current and proficient. Triage patients in accordance with protocols established by the provider. May perform supervisory duties as assigned by leadership. Demonstrating specific nursing tasks or activities according to the office practice's established procedures. Observing the unlicensed assistive personnel return demonstration of specific tasks or activities in comparison to the office practice's established step-by-step procedure. Provides physician with feedback relative to the unlicensed assistive personnel on-going performance of delegated activites. Completes other duties to support departmental operations as assigned or requested. Assists in maintenance of patient medical record. Enters accurate documentation in practice management and applicable computer software. Assists with transmission and coordination of physician orders to appropriate staff and departments. Completes insurance forms, disability forms, patient inquiry calls, pharmacy calls, and make appointments with other medical providers on behalf of the patient. Required Education: Graduate of accredited LPN program Required License: Must have and maintain a current North Carolina or other compact state LPN license BLS Certification required within 90 days of employment. Preferred Experience: Minimum two (2) years experience working in an ambulatory clinic.. Possess an ability to function in a high stress, physically demanding setting.
